Forms

The following are all of the forms that will help make your trip smooth and allow us to have your printed tickets and rental equipment waiting for you.

• Group Leader Arrival Information that will assist you on your first day at Eldora.

Please make copies as necessary for the following:

• Ticket Order Form and Rental and Lesson Reservation Form must both be filled out and faxed at least 7 days prior to your scheduled trip.
• Rental Release Form must be filled out by anyone renting equipment from Eldora Mountain Resort. Please bring these with you at time of your arrival.
• Ski School Release Form must be filled out by anyone taking ski or snowboard lessons at Eldora Mountain Resort. Please bring these with you at time of your arrival. See our lesson descriptions.
• Medical Release Form must be filled out by anyone under the age of 18 and signed by a parent or a guardian. Please bring these with you at time of your arrival
• Group Menu contains the breakfast and/or lunch choices for those who have purchased a meal coupon (please indicate on ticket order form).
• Base Map of Eldora Mountain Resort showing the location of the Group Sales office, Groups Rentals and all other buildings.
