The following are all of the forms that will help make your trip smooth and
allow us to have your printed tickets and rental equipment waiting for you.

Leader Arrival Information a detailed information sheet that will assist you
on your first day at Eldora.
Please make copies as necessary for the following:
3. Ticket
Order Form / Rental
and Lesson Reservation Form these must both be filled out and faxed at
least 7 days prior to your scheduled trip.
4. Rental
Release Form must be filled out by anyone renting equipment from Eldora Mountain
Resort. Please bring these with you at time of your arrival.
5. Ski
School Release Form must be filled out by anyone taking ski or snowboard lessons
at Eldora Mountain Resort. Please bring these with you at time of

6. Medical
Release Form must be filled out by anyone under the age of 18 and signed by
a parent or a guardian. Please bring these with you at time of your arrival
7. Group
Menu contains the breakfast and/or lunch choices for those who have purchased
a meal coupon (please indicate on ticket order form).
8. Base
